What doctor can do plaquenil baseline test

The prescription from that eye exam has expired, and I need a new exam to get new glasses. Called my Rheumy to see if he'd give me a recommendation for an opthalmalogist (I try to keep all my treatment at Vanderbilt Med Center so all my doctors can access my charts).

What does Plaquenil/hydroxychloroquine do to your eyes? The retina is the film in the back of your eyes. CQ and HCQ can both cause maculopathy or retinopathy. Prior to modern diagnostic techniques, your eye doctor was looking for bull’s eye maculopathy with a dilated exam. I am lucky as a rheumatologist in practice for more than 30 years to have only had one malpractice lawsuit. But it was for alleged Plaquenil related eye toxicity, and believe me, attornies will grab any guideline they can as "Standard-of-Care" if it suits their case. All to say, we treat patients, not guidelines, but we all need to aware of these. Utterback’s rheumatologist also referred her to neuro-ophthalmologist David Katz, MD, in Bethesda, Md. “We don’t know which patients might develop Plaquenil retinopathy, but the patient often can see it before the doctor can,” he says. Here are some important warning signs

I kept my temper and explained that I was under the impression I'd need that eye appointment because of the Plaquenil and instead of paying for an optician exam as well, I had hoped to kill two birds with one stone. The nurse called back and gave me tremendous attitude - "your chart says nothing about an eye exam" and "I don't see why he'd give you a referral if it is not in your chart." Soooo...

  5. Apr 20, 2011 A dilated fundus examination is critical in detecting and documenting established macular or retinal disease; however, due to low sensitivity, it should not be used as a screening test alone. Although fundus photography can be used for baseline documentation of retinal disease or to evaluate apparent subsequent changes, it is not sensitive.

The results of the tests may be assessed after you. have left the clinic. When they have been assessed a report will be sent to you, your GP and/or the hospital doctor who prescribed hydroxychloroquine. No signs of retinopathy. If the tests do not show signs of retinopathy, you will be invited back for re-screening in one year. Although the latest guidelines suggest that Amsler grid, color vision and fundus photography are not considered necessary for screening patients on Plaquenil, they can still provide useful information. What tests do you need to perform during their baseline screening? If signs of early toxicity are detected, your doctor may suggest to your rheumatologist to stop Plaquenil and switch to a different medication for your systemic illness. Sometimes the toxicity is not reversible. Patients taking the drug should have an early baseline assessment of visual acuity, macular appearance and central field sensitivity.
